Registered Nurse (Home Health Care)

Position Overview

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team of healthcare professionals. The RN will play a vital role in providing comprehensive medical care to our clients, ensuring their safety, comfort, and well-being in a home setting.

Responsibilities
  • Conduct thorough assessments of patients' health status, including physical, psychological, and social factors.
  • Develop and implement individualized care plans based on patients' needs, goals, and preferences.
  • Administer medications, treatments, and procedures as prescribed by physicians.
  • Monitor patients' vital signs, symptoms, and progress, and make necessary adjustments to care plans.
  • Educate patients and their families on health management, medication adherence, and disease prevention.
  • Collaborate with physicians, therapists, and other healthcare professionals to coordinate and optimize patient care.
  • Document all patient interactions, assessments, interventions, and outcomes accurately and timely.
Qualifications
  • Current, unrestricted RN license in New Jersey.
  • Minimum of 1 year of nursing experience, preferably in home health care or a related field.
  • Knowledge of home health care regulations, practices, and standards of care.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in electronic medical record (EMR) systems.

Required documentations
  • Health attestation form, reflecting the results for Mantoux tuberculin skin test(TB) as well as rubella and rubeola(1957 or later) immunization or serologic evidence of immunity(blood test).
  • Proof of Hepatitis B vaccine (bloodwork/health attestation form) or declination of vaccine.
  • Annual Physical Exam by an MD, within 12 months of employment start date.
    Recent copy of your driver’s license report within the last 12 months.
  • Motor vehicle insurance.
  • Any certifications or degrees you may have earned, including CPR.
  • Minimum three professional references.
  • Background check.
